MAIL ORDER FORM

This order form allows you to  make a purchase by Money Order for  those who wish to NOT use PayPal .

It may also serve my  International Canadian Customers who wish to get a shipping  amount  before submitting payment on line.

How it works..

  I will accept money orders (U.S. ONLY )  if you would like to order by mail  so please fill out the following form below with a brief description of the item/items you would like to purchase. When finished simply submit the form by clicking on the submit button below and the form will automatically be sent to my e-mail address . I will then  send a response to the e-mail  address you have provided  with the total purchase amount  including  shipping cost and payment address information..  Once I receive your payment in the mail I will  process your order as normal and send e-mail notification  of your shipment. If you have trouble with this form or have any questions  please contact me at velvetmoonbathery@hotmail.com.
